Dear Valued NYMC Family...

We were just informed by our scheduling company that  your enrollment form must have either dropped off your account or that it wasn't filled out fully. However, we must keep this form on file for every NYMC student enrolled to make sure that we have emergency contact information (allergies, etc.) for all students and remain in compliance.

Please take a minute to update your information with these easy steps before your next lesson:

2. Enter your student's name and email

3. Add any missing information (you do not need to add payment information as it's already on file.
